For 24 years, ChapCare has been working tirelessly to provide healthcare equity and access for everyone in the San Gabriel Valley, working on-the-ground to help communities thrive.
ChapCare has grown from a small, grassroots community clinic to a network of federally-funded community health centers capable of providing innovative services at a high level. During the pandemic, ChapCare has provided COVID-19 testing and vaccination, and expanded its telehealth services, making medical care more accessible and convenient to its patients.
Today, ChapCare provides more than 68,000 primary healthcare visits annually for the communities it serves.
This health center received HHS funding and has Federal Public Health Service (PHS) deemed states with respect to certain health or Health-related claims, including medical malpractice claims for itself and its covered individuals. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). ChapCare is a 501(c)(3) nonprofit organization dedicated to providing excellent, comprehensive and innovative health care that is accessible to all the residents of the San Gabriel Valley.
